Luis A. Dehesa, MD

Dr. Luis A. Dehesa is a board-certified dermatologist and Mohs micrographic surgeon with more than two decades of experience practicing in both Spain and the United States. His dual board certifications in dermatology and Mohs surgery represent a rare and distinguished achievement in the region. Dr. Dehesa earned his medical degree from the Universidad de Las Palmas de Gran Canaria in Spain in 2004, where he also completed his initial dermatology training and obtained board certification. In 2009, he relocated to the United States with his wife, a Fresno native, to continue advancing his career. In the U.S., Dr. Dehesa completed an internal medicine internship at Mount Sinai Medical Center in Miami, followed by a dermatology residency at UT Health San Antonio. He also served as a research fellow at the University of Miami Hospital, leading clinical trials in psoriasis, lupus, and onychomycosis. After completing his second dermatology residency in 2016, Dr. Dehesa returned to Fresno with his family, where he served as a Clinical Professor of Dermatology at UCSF Fresno for nearly a decade. He is widely recognized for his expertise in general dermatology, surgical dermatology, and Mohs micrographic surgery. Fluent in both English and Spanish, Dr. Dehesa is known for his compassionate, patient-centered approach and his commitment to providing clear, trustworthy care. Dr. Dehesa has authored more than 20 peer-reviewed journal articles, contributed book chapters, and presented at national and international conferences. He is a Fellow of the American Academy of Dermatology, the American Society for Dermatologic Surgery, and the American Board of Dermatology, as well as a member of the Fresno-Madera Medical Society. Milena Lopes Cavalcante, MD, is board-certified in internal medicine and rheumatology. She diagnoses and treats patients with musculoskeletal disease and autoimmune conditions such as rheumatoid arthritis, lupus, gout and osteoarthritis.
Dr. Cavalcante earned her medical degree from the Universidade Federal do Estado do Rio de Janeiro in Brazil and finished her postdoctoral training in Boston. Here she completed an internal medicine residency at Mount Sinai Medical Center and fellowship in rheumatology at Tufts Medical Center. While at Tufts, she also attended the university’s Sackler School of Graduate Biomedical Sciences, receiving certification in clinical and translational science.
Dr. Cavalcante moved to the Central Valley with her family in 2021. She has been working at Adventist Health at Fowler since. Dr. Cavalcante is thrilled to join our team, where she will continue her commitment to excellence in patient care, medical education, and research, welcoming new patients of all ages.
She is a member of the American College of Rheumatology and also has published her work in peer-reviewed journals and presented frequently. She is fluent in English, Spanish and Portuguese.
Duncan Mackay, MD
Board-Certified Plastic Surgeon
Dehesa Dermatology is thrilled to welcome Dr. Duncan Mackay, a distinguished board-certified plastic surgeon, to our team of specialists. Dr. Mackay brings exceptional expertise in plastic, reconstructive, and craniofacial surgery, along with an extraordinary academic and leadership background that enriches the high-quality care we provide to our patients.
Dr. Mackay currently serves as a Pediatric Plastic and Craniofacial Surgeon and Director of Orthognathic Surgery at Valley Children’s Hospital, where he cares for some of the most complex craniofacial conditions in the Central Valley. His advanced training includes a prestigious craniofacial surgery fellowship at Stanford University and a BioDesign Faculty Fellowship through the Stanford Byers Center for BioDesign, where he received specialized training in healthcare innovation and medical technology development.
Dr. Mackay completed his integrated residency in Plastic and Reconstructive Surgery at the Hospital of the University of Pennsylvania and the Children’s Hospital of Philadelphia. He earned his medical degree at the Perelman School of Medicine at the University of Pennsylvania and simultaneously completed an MBA with a specialization in Healthcare Management at the Wharton School.
With a strong foundation in research, Dr. Mackay has published extensively in peer-reviewed journals and has presented his work nationally and internationally. His research includes contributions to craniofacial surgery, wound healing, facial paralysis, breast reconstruction, and global health. He has participated in international surgical missions and co-founded Friends of Noah USA, a nonprofit organization supporting children in South Africa affected by the HIV/AIDS epidemic.
Known for his thoughtful, compassionate care and collaborative approach, Dr. Mackay is committed to delivering exceptional outcomes tailored to each patient’s needs. David Mapes, PA-C
Physician Assistant – Medical & Surgical Dermatology
PA Dave is a board-certified Physician Assistant specializing in medical and surgical dermatology with more than two decades of healthcare experience and a strong background in emergency medicine. A proud graduate of San Joaquin Valley College, he graduated salutatorian of his class in 2015 and was a member of the winning teams for both the California and American Academy of PAs’ Challenge Bowls. He later earned a Master of Science in Physician Assistant Studies from A.T. Still University, graduating with a perfect 4.0 GPA, and is currently pursuing his Doctorate in Medical Science (DMS) at Shenandoah University.
Before transitioning to his role as a Physician Assistant in emergency medicine and dermatology, David served the community as a Paramedic and later a Firefighter from 2002 to 2015. During this time, he also taught American Heart Association lifesaving courses and continues to maintain instructor certifications in Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS).
Dedicated to exceptional patient care, Dave provides compassionate, individualized dermatologic treatment for patients of all ages.
Outside the clinic, David enjoys traveling and creating memories with his wife and two children through simple pleasures like park hopping and discovering the best local food trucks together.
Sheila Mayo, PA-C, MMSc
Physician Assistant Medical Dermatology
Sheila Mayo is a board-certified dermatology physician assistant with more than two decades of experience in medical dermatology. She earned her undergraduate degree from the University of California, Berkeley, and her Master of Medical Science from Emory University in Atlanta, Georgia.
Sheila began her career at Emory University, where she provided patient-centered dermatologic care while mentoring students and serving as a clinical preceptor for 10 years. In recognition of her dedication to teaching and mentorship, she was honored as Outstanding Physician Assistant of the Year in 2011 by Emory’s students and faculty.
Driven by a passion for serving her community, Sheila returned to her home state of California and has spent the past 12 years caring for patients throughout the Central Valley. Outside of her clinical practice, Sheila enjoys participating in a monthly book club and spending time outdoors tending her garden. Above all, she cherishes quality time with her family and close friends.
